The NCLEX-RN is the licensing examination every graduate of a registered-nursing program in the United States must pass to be licensed as a registered nurse. NCSBN builds and scores it; each state board of nursing decides who is eligible and issues the license. The exam is a computerized adaptive test: the computer selects each item based on your earlier answers, you receive between 85 and 150 items, and you have five hours in total regardless of how many items you receive. The test is scored pass/fail. Items follow the 2026 RN Test Plan, which groups content into eight client-needs categories: Management of Care (18%), Safety and Infection Prevention and Control (13%), Health Promotion and Maintenance (9%), Psychosocial Integrity (9%), Basic Care and Comfort (9%), Pharmacological and Parenteral Therapies (16%), Reduction of Risk Potential (12%) and Physiological Adaptation (14%). The registration fee for U.S. licensure is US$200, and NCSBN has announced that it rises to US$350 on February 1, 2027. NCSBN's 2025 statistics show 192,916 first-time U.S.-educated candidates tested for U.S. licensure and 167,272 passed, an 86.7% pass rate.
