The Care of the Extremely Low Birth Weight Neonate (C-ELBW) credential is a subspecialty certification awarded by the National Certification Corporation (NCC). To apply you need a current, active and unencumbered licence in the United States or Canada as a physician (MD or DO), registered nurse, advanced practice registered nurse (NNP or CNS), physician assistant, respiratory therapist, neonatal therapist (occupational therapist, physical therapist or speech-language pathologist), neonatal dietitian, neonatal pharmacist or paramedic. NCC requires no practice experience but recommends at least one year of experience caring for extremely low birth weight infants. NCC's 2026 Candidate Guide lists the computer exam fee, for either a test center or live remote proctoring, as $210, which includes the non-refundable $50 application fee; NCC states that examination fees are subject to change. After approval you have a 90-day eligibility window to test, and NCC advises scheduling within the first 30 days of that window. A change request costs $125, a candidate who withdraws receives $105 of the $210, and a retest requires the full application and examination fees again.
