ABIM Internal Medicine exam requirements & cost

The ABIM Internal Medicine Certification Exam is open only to physicians who have completed an ACGME-accredited internal medicine residency — 36 months of training total, at least 30 months of which must be clinical. There is no separate application beyond ABIM's own certification enrollment process, but candidates must be in good standing with their residency program and meet ABIM's licensure and professionalism requirements. As of 2026, the initial certification application fee is approximately $1,690; ABIM also runs a needs-based fee-assistance program offering a 50% credit to eligible first-time candidates, with applications typically open in the fall before the exam. The exam itself is administered at Pearson VUE Professional Centers as up to 240 single-best-answer multiple-choice questions (some unscored) split across four ~2-hour modules, for a full exam-day appointment of roughly 10 hours including breaks.
