Unintentional weight loss of this magnitude and speed — roughly 5% or more within a month — is a recognized nutrition-risk indicator regardless of starting BMI, because it reflects an active, ongoing process of tissue loss that a single point-in-time BMI cannot capture. A patient can be losing significant lean and fat mass while their BMI number still reads in the 'overweight' range simply because they started from a higher baseline; the rate and direction of change matters as much as the static number. Relying on BMI alone would miss this patient's genuine and clinically significant risk, delaying intervention until the weight loss has progressed further and become harder to reverse.
