CCS Exam Pass Rate, Format & Study Guide
A real reference for anyone preparing for the Certified Coding Specialist (CCS) Exam — AHIMA exam — pass rate, retake policy, how it's scored, and a practical study timeline. This is independent reference content, not a sales page: everything below is sourced from CCS Exam's own certifying body.
What the CCS Exam exam is
The CCS exam has 107 questions, of which 97 are scored and 10 are pretest, and candidates have four hours; the exam is computer-based at a Pearson VUE test center, in the US and internationally (AHIMA CCS page). AHIMA states that the passing score for the CCS is 300 on its scaled scoring. The exam content outline describes multiple-choice items plus medical scenarios drawn from inpatient, outpatient and emergency department records, in roughly equal thirds (CCS Exam Content Outline, effective May 1, 2024).
Why it matters
The CCS focuses on inpatient and outpatient hospital coding and is one of the best-known credentials for coders working in hospitals and health systems.
Who this exam is for
Medical coders and coding students working toward hospital inpatient and outpatient coding roles.
Pass rate
The certifying body does not publicly publish an official pass rate for this exam. Be wary of third-party sites citing a specific number here — treat any pass-rate claim you see elsewhere for this exam as unverified.
If you don't pass — retake policy
To retake the CCS you submit a new application with the exam fee and wait at least 30 days before the application can be approved; AHIMA states that it cannot waive the waiting period (AHIMA CCS page).
How it's scored
AHIMA reports the CCS as a scaled score and sets the passing score at 300. A scaled score is not a percentage of questions answered correctly, so the portal mock shows 70 percent only as a self-check. Format: 107 items in four hours, 97 scored and 10 pretest, multiple-choice plus medical scenarios. No official pass rate is available for this exam, so none is quoted here.

What each domain actually tests
AHIMA groups the content into coding knowledge and skills, coding documentation, provider queries, regulatory compliance and information technologies. The real exam adds medical scenarios (inpatient, outpatient and emergency department, about one third each) that are worked with code books and are not simulated in the portal.
Eligibility requirements, in full
AHIMA recommends, rather than requires, experience or credentials. Options include two years of related coding experience, or a CCA, another coding credential, or a CCS-P, RHIT or RHIA plus one year of direct coding experience, or a course-based path with one year of coding experience.
What it actually costs
$299 for members and $399 for non-members; a retake needs a new application and the full exam fee.
Keeping the credential — maintenance & recertification
AHIMA states that the credential runs on a two-year certification period with continuing education units and a recertification fee; confirm the current CEU count on ahima.org.
How to actually prepare
Work through the exam topics one area at a time, finish with timed full-length practice runs of 97 questions in one sitting.
What's changing (2026)
AHIMA updates code books each year and lists a content outline effective May 1, 2024; this guide follows the pages listed under Verification as of October 2026.
